Las fosfatasas alcalinas (FA) son un grupo de 4 isoenzimas que se producen en diversos tejidos, pudiendo elevarse en condiciones fisiológicas y secundariamente a enfermedades óseas o hepatobiliares. En niños una de las causas más frecuentes e inocuas, pero poco conocidas, es la hiperfosfatasemia transitoria benigna de la infancia (HFTBI). El objetivo es reportar una serie de casos de HFTBI y proponer un criterio de enfrentamiento. Casos clínicos: Se presentan 5 niños de entre 11 y 50 meses de edad, 4 de ellos de sexo femenino, con hallazgo incidental de elevación severa (> 1.000 UI/l) en los niveles de FA en exámenes solicitados por mal incremento pondo-estatural o por cuadros infecciosos. A través de la anamnesis, examen físico y laboratorio básico se descartó enfermedad ósea o hepática. En 2 de los pacientes se determinaron las isoenzimas, destacando el predominio óseo. Se comprobó una normalización de los niveles de FA en un periodo de uno a 6 meses, sin evidencia de complicaciones posteriores. Conclusión: La HFTBI es un desorden bioquímico benigno de evolución autolimitada, que es importante tener presente al enfrentar un niño menor de 5 años con elevación severa de FA, en ausencia de alteraciones clínicas o de laboratorio que sugieran enfermedad ósea o hepática.


Alkaline Phosphatase (ALP) is a group of 4 isoenzymes produced in different tissues. Elevated levels of ALP can be developed under physiological conditions, and can indicate the presence of bone or hepatobiliary diseases. In children, one of its most common harmless causes is benign transient hyperphosphatasaemia (BTH), a little known condition. The objective is to report BTH cases and propose a monitoring plan. Case reports: The cases of 5 children aged between 11 and 50 months are presented, 4 of them female, with the incidental finding of a sudden and severe ALP elevation (> 1,000 U/l), in tests ordered due to either abnormal growth and development, or intercurrent infections. Bone and liver disease were ruled out using the patient history, physical examination and basic laboratory results. Isoenzymes levels were determined in 2 patients. A return to normal ALP levels was observed over a period of 1-6 months, with no evidence of further complications. Conclusion: BTH is a benign self-limiting biochemical disorder, which should be considered in children under 5 years old with severe ALP elevation in the absence of clinical or laboratory abnormalities suggestive of bone or liver disease.

CONTEXT AND OBJECTIVE: Heparanase-1 degrades heparan sulfate and has been correlated with tumor progression. Although the isoform heparanase-2 has no catalytic activity, it seems to be important for modulating heparanase-1 activity. Cathepsin B is a proteinase involved in tumor metastasis. The aim of this study was to analyze heparanase isoform expression and cathepsin B activity in plasma samples from patients with gastrointestinal carcinomas, compared with healthy individuals (control group). DESIGN AND SETTING: This was an analytical cross-sectional study. Peripheral blood samples were collected at a Brazilian public hospital, from 21 patients with histopathological diagnoses of gastrointestinal carcinomas and from 43 healthy individuals. The analyses were performed in two Brazilian medical schools. METHODS: Heparanase isoforms were identified and quantified in plasma samples by means of Western blot. The enzymatic activities of heparanase-1 and cathepsin B were also measured. RESULTS: The results demonstrated that the expression of both heparanase isoforms was significantly greater in plasma samples from gastrointestinal carcinoma patients, compared with the control group. CONTEXTO E OBJETIVO: A heparanase-1 degrada heparam sulfato e está relacionada à progressão de tumor. Apesar de a isoforma heparanase-2 não possuir atividade catalítica, parece ser importante para modular a atividade da heparanase-1. A catepsina B é uma proteinase envolvida na metástase de tumores. O objetivo deste estudo foi analisar a expressão das isoformas de heparanase e atividade da catepsina B em amostras de plasma de pacientes com carcinomas gastrointestinais, comparando-se com indivíduos saudáveis (grupo controle). TIPO DE ESTUDO E LOCAL: Este é um estudo transversal analítico. Foram coletadas amostras de sangue periférico, em hospital público brasileiro, de 21 pacientes com diagnóstico histopatológico de carcinoma gastrointestinal e 43 indivíduos saudáveis. As análises foram realizadas em duas faculdades de medicina brasileiras. MÉTODOS: As isoformas da heparanase foram identificadas e quantificadas em amostras de plasma por Western blot. As atividades enzimáticas de heparanase-1 e catepsina B foram também mensuradas. La fosforilación de la glucosa en los mamíferos, es catalizada por una familia de isoenzimas (hexoquinasas I-IV; HQ) de diferente Km para el azúcar. En los hepatocitos y células b-pancreáticas se encuentra la glucoquinasa (GQ; HQ IV) de Km elevado (12-20 mM). Hemos observado que GQ está presente en el intestino delgado y podría contribuir a la producción de lactato durante la absorción del azúcar. En este trabajo se determinó el efecto de la dieta (ratarina R; 60% de glucosa G; sacarosa S; almidón A; caseína C), suministrada ad libitum, sobre las actividades de HQ y GQ en homogenatos de hígado y mucosa intestinal de rata. El suministro de glucosa (5%) en el agua de beber (SG) también fue evaluado en las dietas con R y G. Las actividades de HQ (Glucosa 1 mM) y la capacidad fosforilativa total (CFT: Glucosa = 100 mM) se determinaron enzimáticamente. GQ se estimó por diferencia. En el grupo control (R) y en S, A y C, la GQ hepática fue un 85% de la CFT, mientras que en G, GSG y RSG un 66%. La HQ intestinal alcanzó en los grupos R, GSG, A y C un 87% y en RSG un 30% de la CFT. La GQ en G, S, aumentó, pero una menor magnitud. La presencia de GQ en el intestino delgado y su expresión diferencial de acuerdo a la dieta, abren la posibilidad de que dicho órgano contribuya al metabolismo inicial de la glucosa procedente de la dieta y provea al hígado de un precursor (lactato) muy eficaz para sus procesos anabólicos.


Glucose phosphorylation in mammals, is catalyzed by a family of isoenzymes (hexokinases I- IV; HQ) of different Km for the sugar. In hepatocytes and pancreatic b- cells are glucokinase (GQ ; HQ IV) of high Km (12-20 mM). We observed that GQ is present in the small intestine and may contribute to the production of lactate during the absorption of sugar. In this work, the effect of diet (ratarina R, G 60% glucose, sucrose S; starch A; casein C) provided ad libitum , on the activities of HQ and GQ in liver homogenates of rat intestinal mucosa . The supply of glucose (5%) in the drinking water ( SG ) was also evaluated in the diets with R and G. HQ activities (Glucose 1 mM) and phosphorylating full capacity ( CFT : Glucose = 100 mM ) were determined enzymatically . GQ was estimated by difference. In the control group (R) and S, A and C, the GQ liver was about 85% of CFT, whereas G, GSG and RSG 66%. The intestinal HQ reached in the R groups, GSG, A and C by 87% and 30% RSG the CFT. The GQ in G, S , increased , but a lower magnitude. the presence of glucokinase in the small intestine and its differential expression according to diet, open the possibility that this structure contributes to initial metabolism of glucose and provide to the liver a precursor (lactate) very effective for their anabolic processes.

Com o objetivo de estudar a influência do exercício físico prolongado na indução de lesão miocárdica e na desqualificação de eqüinos em provas de enduro, colheram-se 87 amostras de sangue de eqüinos adultos Árabes e mestiços para determinação da concentração sérica da isoenzima MB (CKMB), da atividade sérica de creatina quinase (CK) e do índice CKMB/CK. Dividiram-se as amostras da seguinte forma: C (controle, n=34): eqüinos em repouso, no dia anterior à competição; G1 (n=24): eqüinos que finalizaram as competições; G2 (n=14): eqüinos desqualificados durante as competições por causa metabólica e G3 (n=15): eqüinos desqualificados durante as competições por claudicação. Os valores medianos obtidos de CKMB, para os grupos C, G1, G2 e G3, foram respectivamente 2,25; 2,95; 1,95 e 2,40 ng/mL; para a atividade de CK, os resultados foram 164,2; 1228,0; 840,0 e 581,0 U/L a 30oC; e para o índice CKMB/CK, os resultados foram 1,41; 0,21; 0,23 e 0,42%. Houve média correlação positiva entre distância percorrida e CKMB e CK, e negativa entre distância e índice CKMB/CK. O exercício físico de resistência não leva a aumento significante dos valores da isoenzima CKMB, mas sim da atividade sérica da enzima CK, indicando dano à musculatura esquelética. Animais desqualificados por causa metabólica parecem não apresentar envolvimento de lesão miocárdica na causa de desqualificação. Não foi possível verificar a presença de lesão cardíaca induzida pelo exercício em eqüinos de enduro e sua relação com causas metabólicas de desqualificação a partir da utilização de CKMB como único marcador cardíaco.


With the purpose of studying the influence of prolonged phyisical exercise causing myocardial lesion and disqualifying horses in endurance competitions, 87 blood samples were collected from adult Arabian and crossbred horses, to determine the serum concentration of the isoenzyme MB (CKMB), the serum activity of creatine kinase (CK), and the CKMB/CK index. Samples were divided as follows: C (control, n=34): horses at rest, one day before competition; G1 (n=24): horses which finished competitions; G2 (n=14): horses disqualified during competitions by metabolic causes, and G3 (n=15): horses disqualified during competitions after lameness was diagnosed. Median values obtained for CKMB, for groups C, G1, G2, and G3, were, respectively, 2.25; 2.95; 1.95, and 2.40 ng/mL; for CK activity, the results were 164.2; 1228.0; 840.0, and 581.0 in U/L at 30oC; and for CKMB/CK index, the results were 1.41; 0.21; 0.23, and 0.42%. There was mean positive correlation between distance and CKMB and CK, and negative correlation between distance and CKMB/CK index. It was concluded that endurance exercise does not cause significant increases in CKMB values, but in CK serum activity, indicating skeletal muscle injury. Horses disqualified by metabolic causes seem not to present myocardial injury involved in the cause of disqualification. It was not possible to verify the presence of exercise-induced cardiac injury in endurance horses and its relationship with metabolic causes of disqualification only by the utilization of CKMB as the only cardiac marker.

PURPOSE: Liver ischemia-reperfusion injury is a phenomenon presents in events like liver resections and transplantation. The restoration of blood flow may leads to local and systemic injury. Several techniques have been developed in order to avoid or ameliorate ischemia-reperfusion injury in clinical situations. The application of a sttuter reperfusion after the ischemic event (postconditioning) could alters the hydrodynamics and stimulates endogenous mechanisms that attenuate the reperfusion injury. The present study was designed to evaluate the potential protective effect of postconditioning in a model of ischemia-reperfusion in rats. METHODS: Hepatic anterior pedicle of median and left anterolateral segments were exposed and clamped for 1 hour. Two hours later, clamp was released in two different ways: Control Group (n=7): clamp was release straightforward; Postconditioning Group (n=7): clamp was released intermittently. Lipid peroxidation (malondialdehyde) and expression of the glutathione-s-transferase-α-3 gene were studied. RESULTS: Lipid peroxidation was significantly decreased in ischemic and non-ischemic liver by postconditioning. GST- α3 gene was overexpressed in postconditioned group, but not significantly. CONCLUSION: Postconditioning induced hepatoprotection by reducing lipid peroxidation in the ischemic and non-ischemic liver.


OBJETIVO: A lesão de isquemia-reperfusão hepática é um fenômeno presente em eventos tais como ressecções hepáticas e transplante de fígado. A restauração do fluxo sangüíneo após a isquemia gera lesões locais e sistêmicas. Várias técnicas foram desenvolvidas com o objetivo de evitar ou diminuir a lesão de isquemia-reperfusão hepática em situações clínicas. A utilização da reperfusão intermitente após o evento isquêmico (pós-condicionamento) pode alterar a hidrodinâmica e estimular mecanismos endógenos que atenuam o dano da reperfusão. O presente estudo foi realizado para avaliar o potencial efeito protetor do pós-condicionamento em um modelo de isquemia-reperfusão em ratos. MÉTODOS: O pedículo dos lobos mediano e ântero-lateral foi isolado e clampeado por 1 hora. Após 2 horas, o pedículo foi liberado de duas maneiras diferentes: Grupo Controle (n=7): clampe liberado de uma só vez; Grupo Pós-condicionamento (n=7): clampe liberado de maneira intermitente. Malondialdeído (MDA) e expressão do gene GST- α3 foram estudadas nos grupos. RESULTADOS: A peroxidação lipídica foi significativamente diminuída no fígado isquêmico e no fígado não isquêmico pelo pós-condicionamento. A expressão do gene GST- α3 aumentou, porém não significativamente, no grupo pós-condicionamento. CONCLUSÃO: O pós-condicionamento induziu hepatoproteção pela redução da peroxidação lipídica nos fígados isquêmico e não isquêmico.

The aim of the present study was to determine the relationship between histopathological lesions and tissue enzymes in chickens affected by Marek's disease.Five apparently healthy chickens [as control group] and 25 chickens affected by Marek's disease were selected. After consideration of history and gross lesions, tissue samples were collected from kidney, liver, heart, ovary, pectoral muscle, spleen, crop, proventriculus and bursa of fabricius for histopathological and tissue enzyme studies. In tissue samples, the activity of aspartate aminotransferase [AST], alkaline phosphatase [ALP], lactate dehydrogenase [LDH], creatine kinase [CK], superoxide dismutase [SOD] and glutathione peroxidase [GPX]was measured. Histopathologically, lymphomatous lesions in visceral organs were included in local or diffuse ple omorphic small to medium lymphocytes, lymphoblasts, marek cells and rarely plasma cells. The results indicated that the activities of AST, LDH, CK, SOD and GPX were decreased in the crop of affected chickens. The activities of CK, LDH and AST in the liver tumors and the activities of LDH, AST, ALP, SOD, CK and GPX in pectoral muscles were increased. In proventriculus, the activities of AST, ALP, LDH and SOD were shown to be increased, but the activity of CK was decreased. The activities of AST and SOD in the ovary lesions and the activities of SOD, AST, CK and LDH in the heart lesions showed an increase. Fluctuations in the activity of enzymes in different tissues showed that measurement of these enzymes can not be used as a tumor marker in the diagnosis of Marek's disease. In order to determine whether these enzymes are tumor markers or not, the measurement of the specific isoenzymes of each enzyme is necessary

RESUMO

Macroenzymes are normal enzymes complexed with an immunoglobulin (usually IgG, rarely IgA or IgM). A number of macroenzymes have been reported in the literature. Among them, macro-AST has been detected in diseases such as acute and chronic hepatitis, various malignancies and autoimmune diseases, but usually not associated with any specific disease. We report a case of elevated AST activity in serum due to marco-AST formation in a female with chronic hepatitis C which was confirmed by AST isoenzyme electrophoresis. To our knowledge, this is the first report of macro-AST occurred in chronic hepatitis patient in Korea.

OBJETIVO: Verificar o efeito prognóstico das alterações de quatro marcadores de lesão miocárdica (CKMB atividade/massa e troponinas T e I) pós- stent em relação à ocorrência de morte, infarto e novos procedimentos de revascularização em um ano e determinar a incidência e as variáveis preditoras de suas elevações. MÉTODOS: Em 199 pacientes tratados por stents em artérias naturais foram mensurados os quatro marcadores em três períodos: pré-procedimento, 6-8h e 14-18h pós-stent. Curvas de sobrevivência e a regressão logística de Cox determinaram o impacto prognóstico das alterações na ocorrência de eventos cardíacos em um ano. RESULTADOS: A incidência das alterações dos 4 marcadores pós-stent é relativamente freqüente (6,1 por cento a 32,8 por cento), associa-se à maior complexidade das lesões tratadas (longas e angulações >45º) e ao desenvolvimento de intercorrências durante o procedimento (oclusão de ramos secundários, presença de angina e alterações eletrocardiográficas). A sobrevivência livre de infarto e nova revascularização foi significativamente menor somente nos pacientes com elevação da CKMB atividade pós-procedimento em comparação àqueles sem esta alteração (60 por cento x 85,08 por cento, p=0,025). Influenciaram a sobrevivência livre de eventos o diabetes mellitus (OR: 2,27, p=0,0256), pré-dilatação com balão (OR: 3,16, p=0,0082) e elevação da CKMB atividade pós-procedimento (OR: 3,64, p=0,0162). CONCLUSÃO: A recomendação da monitoração sistemática da CKMB atividade pós-stent coronário baseia-se na compreensão do seu comportamento clínico e laboratorial, nos resultados dos estudos que relacionam a elevação de seus níveis pós-procedimento à pior evolução tardia e ao baixo custo do exame.

To assess the value of troponin T [TpT] in the coronary care unit [CCU] setting compared to creatinine phosphokinase [CK-MB] in patients admitted with acute coronary syndrome. This was a prospective study conducted over a period of 2 months between May 2003 and June 2003. All patients who were admitted to the CCU at Queen Alia Heart Institute, Amman, Jordan with acute coronary syndrome were included. Troponin T and CK-MB were performed simultaneously on all patients upon admission and serially every 4 hours for 24 hours. The times of the serial measurements from the onset of chest pain and the results were recorded. The result of coronary angiography was recorded in those patients who underwent this procedure during the index hospitalization. Patients with chest pain more than 48 hours prior to admission and those with renal impairment were excluded. One hundred and ninety-seven patients were enrolled in our study. Sixty-one% were males. The mean age was 60 years with a range of 28-90 years. The total number of patients with a positive biomarker [TpT or CK-MB] was 136. Forty-nine patients [36%] had a positive TpT without an accompanying CK-MB leak. Only 2 patients [1.4%] had a CK-MB without a positive TpT. The positive predictive value of TpT was 94%, with a negative predictive value of 96%, giving 98.5% sensitivity and 97% specificity. The earliest time from the onset of pain to having a positive TpT was one hour. Out of the 197 patients 173 [87.8%] had cardiac catheterization and it did not seem to have been affected by a negative TpT or CK-MB. There were 5 deaths, and their TpT results were well above the average positive value. Troponin T is a more sensitive and specific biomarker than CK-MB in detecting myocardial injury. It can become positive as early as one hour from the onset of chest pain. The decision whether to do coronary angiography remains based on clinical assessment rather than laboratory data

OBJECTIVE@#To study the changes of serum creatinine kinase(CK) and its cardiac-specific isoenzyme compound(CK-MB) levels in crush injury rats.@*METHODS@#Crush injury was produced in SD rats, the serum levels of CK and CK-MB were studied by automated biochemical analyzer.@*RESULTS@#The levels of plasma CK and CK-MB were much higher in crush injury rats than those of the control group.@*CONCLUSIONS@#Cardiomyocyte injury may be induced in the early stage of crush injury rats.

COX-2 protein, but not COX-1 protein, was induced in HUVEC from women with a normal pregnancy (nHUVEC) treated with serum from patients with preeclampsia (pSerum), but not with serum from women with a normal pregnancy (nSerum). COX activity in pSerum treated nHUVEC was less than in nSerum treated nHUVEC. Interestingly, the induction of COX-2 protein in nHUVEC treated with pSerum was inhibited by antiIL-6 antibody. The decreased COX activity in nHUVEC treated with pSerum plus antiIL-6 antibody was also reversed in a dose dependent manner. Thus, the induction of COX-2 in pSerum treated nHUVEC was mediated by IL-6. Therefore, the development of selective inhibitors of COX-2 or of IL-6 antagonists may have a potential role in the prevention and treatment of preeclampsia.

RESUMO

Macroamylasemia is a condition of persistent, elevated serum amylase activity with no apparent clinical symptoms of a pancreatic disorder. In Korea, however, no such case has been reported to date. We report a case of a 17-year-old female diagnosed with macroamylasemia and acute appendicitis. One day earlier, she developed epigastric and right lower quadrant abdominal pain. She was characterized by high level of serum amylase, but normal lipase. Amylase isoenzyme analysis demonstrated increased fraction of salivary type and follow-up amylase level was persistently increased. Immunofixation disclosed the macroamylase binding with an immunoglobulin, consisting of IgA and kappa chain. The patient was treated by appendectomy, and the abdominal pain subsided.

RESUMO

Aplica-se técnica alternativa, rápida, simples e de baixo custo operacional para a dosagem da isoenzima óssea de fosfatase alcalina baseada na termoinativaçao como método qualitativo e na relaçao de Roy como método quantitativo. Essa técnica foi usada em busca de valores preliminares de normalidade. Foi encontrada média de 39,96 por cento (DP=16,50), como valor normal em indivíduos entre um e 20 anos de idade. A importância da fosfatase alcalina óssea em certas condiçoes ortopédicas patológicas como marcador da atividade osteoblástica é discutida, especialmente em tumores ósseos malignos (osteossarcoma). Conclui-se que, por se tratar de técnica de fácil execuçao, acabará por difundir o uso da isoenzima óssea de fosfatase alcalina na pesquisa da atividade osteoblástica.

RESUMO

Serum total Amylase and its heat stable fraction (HSF) were studied in 10 healthy controls, and 20 patients of Viral hepatitis. A highly significant (p < 0.001) elevation in the levels of total and HSF of Serum amylase was found in acute viral hepatitis, mainly due to the HSF which originates from hepatitis. The levels declined significantly after 10 days when the disease regressed and there was evidence of improvement in clinical and functional status of the liver. Hence an estimation of total and HSF of Serum amylase proved to have some diagnostic and prognostic importance.

Leukocyte acid phosphatase and its isoenzyme composition was studied in leukemic patients to determine the specificity of different isoenzymes in leukemic leukocytes. It was found that leukocyte acid phosphatase content is significantly increased in ALL, AML, and CML patients, while CLL patients had decreased levels of acid phosphatase. The distribution and intensity of leukocyte ACP isoenzymes vary in respective leukemic condition. Thus isoenzyme 'O' was predominant in AML and CML, while isoenzymes 1, 2 and 3 predominated in ALL. The lack of predominance of isoenzyme 3 was a feature in CLL patients. It was concluded that the isoenzyme patterns, though promising, presented inconclusive picture for diagnosis purpose and further studies on immunochemical characteristics of these isoenzymes are warranted to ascertain their cell specificity.

Serum total sialic acid (TSA), Regan isoenzyme (RI) (heat stable placenta like alkaline phosphatase) and TSA/Total protein (TP) ratio were estimated in 44 cases of Lung cancer using highly specific chemical methods to assess their efficacy as malignancy markers. Levels of the two markers were compared with 27 matched healthy controls. A significant increase (P less than 0.001) in both the bio-markers were compared with 27 matched healthy controls. A significant increase (P less than 0.001) in both the bio-markers was observed compared to controls. TSA/TP ratio was significantly higher (P less than 0.001) in lung cancer patients compared to controls. TSA levels were elevated in more cases of patients compared to elevations in RI. Mean values of TSA and RI were higher in epidermoid carcinoma than adeno carcinoma. The use of combination of these markers may be considered in the diagnosis of the disease in future trials.
